  • Full Choice of Plans Anthem BCBS offers 15 different plans to choose from.  This includes 6 PPO plans, 5 Lumenos Consumer-Driven Health Plans (CDHP) and 4 HMO plans.  Offer just one plan or any number of plans that fit your needs best.                                

  • Strong statewide and national provider network (VIEW NETWORK INFO)

  • Financial Control – Anthem offers two options for Employer’s monthly contribution towards your employees’ health premiums. 

    • Fixed-dollar contribution – You pay as little as $125 per employee

    • Traditional contribution – You pay as little as 50% per employee

  • Employer Access (VIEW EMPLOYER ACCESS INFO) allows you to get an overview of your employees’ coverage, enroll employees, add dependents, change or cancel coverage, request ID cards, view a list of open invoices, view and pay bills online and much more.

  • 360˚ Health (VIEW 360° HEALTH INFO) brings together the information and services you need to take good care of yourself and your family and to help you make informed health care decisions. 

  • Additional savings opportunities may be available if you also purchase Life, Dental, Vision, Short Term Disability or Long Term Disability through Anthem

Added Benefits for CBA Members

  • Cost Savings – CBA has negotiated with Anthem to offer discounted rates on Anthem’s Employee Elect Small Group Portfolio.  Small Group is 1 to 50 employees

  • Self-employed “Business Groups of One” (VIEW BG1 INFO) have access to the entire Employee Elect Portfolio with Guarantee Issue. Self-employed individuals must meet the criteria for a “business group of one” as defined in section 10-16-102(6).

  • Member Assistance Program (MAP) (VIEW MAP INFO) Anthem offers a comprehensive MAP at no additional cost; helping reduce absenteeism and benefit expenses while boosting job productivity and healthy lifestyles.

  • Potential tax advantages – Anthem will offer a Premium Only Plan (POP) at no additional charge for the first year.  Take advantage of IRS Section 125 provisions that allows employees to pay their premiums with pre-tax dollars.
